		Health and Dental Change Form Health_Dental_Change_0904_en.pdf \| 41.6 KB Print out this form, complete it, and the submit it to the Supervisor of Payroll at the Chesley Administrative Centre within 30 days of the birth of the baby in order to add the baby to your Extended Health Care Insurance and/or Dental Insurance.
